Overview:

Back pain is the most common reason people walk through our doors in Bella Vista, and no two cases arrive the same way. Some start with a deadlift or a weekend in the garden. Others build slowly over months at a desk. And plenty of people come to us after trying other treatment without a lasting answer. If that is you, you are not starting again from scratch. Anatomy:

The lower back is built around five lumbar vertebrae, separated by discs that absorb load and joined by small facet joints that guide movement. Layers of muscle and ligament support the spine, and nerves exit between the vertebrae to supply the legs. Pain can come from any of these structures, the joints, the discs, the muscles or the nerves, and working out which one is involved is what a thorough assessment is for.

Causes:

Common causes of lower back pain include:

  1. Muscle and ligament strain: lifting, sport, gardening or an awkward movement overloading the supporting tissues. Read more on our back strain page.
  2. Disc-related pain: irritation or injury of a disc, sometimes with pain referring into the buttock or leg.
  3. Facet joint pain: stiff or irritated spinal joints, often worse with extension or prolonged standing.
  4. Nerve-related pain (sciatica): irritation of a nerve root producing leg pain, pins and needles or numbness.
  5. Sustained postures and deconditioning: long hours sitting and low activity gradually reducing the back’s tolerance for load.
  6. Training load spikes: a sudden jump in gym, running or sport volume.

Symptoms:

Signs and symptoms of lower back pain may include:

  1. Aching, tightness or sharp pain across the lower back.
  2. Pain that flares with sitting, bending, lifting or first thing in the morning.
  3. Pain referring into the buttock or leg, sometimes with pins and needles.
  4. Stiffness that eases as you get moving.
  5. Difficulty with sport, work tasks or sleep because of the pain.

Back pain with loss of bladder or bowel control, numbness in the saddle region, or progressive leg weakness needs urgent medical assessment.

Diagnosis:

Diagnosing lower back pain involves a comprehensive evaluation, typically including:

  1. Medical history: how the pain started, how it behaves through the day, your work and training loads, and what treatment you have already tried.
  2. Physical examination: assessing how your spine, hips and movement patterns share load, with neurological testing where leg symptoms are present.
  3. Imaging tests: not needed for most back pain. Where the story warrants it, imaging or a medical opinion is arranged, and we explain why.

Treatment:

Your first visit is a full assessment, not a quick look and a crack. You leave with a report of findings and a plan, and you are under no obligation to continue care. Depending on the findings, treatment may include:

  1. Manual therapy: hands-on care including joint mobilisation and adjustment, and soft tissue work, matched to your presentation.
  2. Exercise rehabilitation: a progressive program to build the strength and control that protect your back.
  3. Advice and education: plain-language explanation of what is driving your pain, what to modify, and what to keep doing. Staying active within limits usually beats rest.
  4. Remedial massage: available at this clinic alongside chiropractic care, including combined appointments.
  5. Referral where needed: where imaging or another opinion is the right call, we say so and refer.

Rehabilitation:

Rehabilitation progresses from settling the current episode to rebuilding the strength, mobility and confidence that keep it from coming back. Programs are matched to what you actually need to do, whether that is lifting at the gym, running, chasing toddlers or sitting through a work day, and they are reviewed and adjusted as you progress. Prevention:

While back pain cannot always be prevented, the following strategies may help reduce the risk:

  1. Keep moving: regular activity and changing position often beat any single perfect posture.
  2. Build strength: a stronger back and hips tolerate more load before complaining.
  3. Progress gradually: increase gym, running and sport loads step by step.
  4. Act early: address flare-ups when they start rather than pushing through for months.

Outlook:

Most back pain settles well with active, conservative care, and even long-standing pain usually improves once the actual drivers are identified and addressed. The course varies from person to person, which is why plans are personalised and reviewed rather than set once. The starting point is a thorough assessment.
