Common shoulder presentations
Rotator cuff tendinopathy
The rotator cuff is a group of four muscles that stabilise the shoulder joint and control its movement. Tendinopathy in any of these muscles — most commonly the supraspinatus, and it can produce pain with overhead activities, lifting, and lying on the affected side at night.
Subacromial pain syndrome
Previously called “impingement,” this term describes pain arising from the structures within the subacromial space (the gap between the top of the shoulder blade and the head of the humerus). Symptoms are typically aggravated by reaching overhead or across the body.
AC joint pain
The acromioclavicular joint sits at the top of the shoulder. It can be injured acutely (often in contact sports or a fall onto the shoulder) or become symptomatic from degenerative change. Pain is typically localised to the top of the shoulder and reproduced by cross-body movements.
Shoulder instability
Instability can range from subtle hypermobility to frank recurrent dislocation. It’s more common in overhead athletes (swimmers, throwers, racquet sports) and in those with hypermobile joints. A thorough assessment is important to distinguish instability from other shoulder pain causes, as management approaches differ.
What does a sports chiropractic shoulder assessment involve?

- Orthopaedic tests to help differentiate between shoulder pathologies

The role of the thoracic spine and scapula
Restricted thoracic mobility — stiffness through the mid-back — is one of the most commonly overlooked contributors to shoulder pain. When the thoracic spine can’t extend or rotate adequately, the shoulder is asked to compensate, placing greater demand on the rotator cuff and subacromial structures.
Similarly, how the scapula moves during shoulder elevation (scapulohumeral rhythm) is clinically important. Altered scapular mechanics can reduce the effective subacromial space and contribute to pain with overhead activities.
